Community Caring Through Corporate Connections identifies the homeless in our community, their needs and some common misconceptions surrounding homelessness in general.  Understanding the role of the Teen Health Centre (THC) in our community is paramount, as one of its programs, Street Health, services individuals outside of the THC’s standard target population: youth ages 12-24.  In fact, the very mantra of the THC is Promoting youth, family and community health through service, community education and advocacy.  The THC is committed to the well-being of our community and a significant aspect of this is raising awareness in our local area about our local area.

 

Our Street Health program provides the in-need population with health care, foot care, dental services and health card/supporting identification replacement services.

 

Our inter-disciplinary team includes a nurse practitioner, consulting physician, foot care nurse, outreach workers and administrative support.

Our wholistic philosophy addresses the total health and wellness of the person. We are caring, concerned and non-judgmental. We respect people’s independence and individuality.

The youth who come to us are homeless, or at risk of being so. Some are living in temporary shelters, motels or rooming houses. They may be living temporarily with friends or family or in substandard housing. Others have been recently released from treatment centres and jails.

In addition to providing health care we also offer:

  • assistance in obtaining Ontario Health Insurance Plan cards, Social Insurance cards, Birth Certificates and certain Immigration documents
  • counselling and case management
  • hair care clinics
  • hygiene and cleaning supplies
  • laundry program
  • footwear assistance
  • dental hygiene clinics
  • screening for Hepatitis B, Hepatitis C, Sexually Transmitted Infections (STIs), TB, etc.
  • Hepatitis C information and assistance
  • outreach and advocacy
  • referrals
